關(guān)于cra的自我介紹

我從事CRA的一點(diǎn)體會(huì)

一年前我已從事臨床工作近五年，考了中級(jí)職稱后改的行。個(gè)中原因主要感到從事醫(yī)生使我顧家太少，盡管我很熱愛這份工作。

醫(yī)生是個(gè)造福后代的職業(yè)，自己得到和付出的即便在升到主治后可能略相稱，可也沒有時(shí)間安排妥自己的生活。

一年前加入了家CRO，進(jìn)公司后我先后參與或負(fù)責(zé)或參與協(xié)助近5個(gè)項(xiàng)目。其中有III期的，IV期的，還有上市后的調(diào)研。Sponsor都是世界藥企前十的幾家。有的項(xiàng)目從選中心開始，組織研究者會(huì)議，遞交倫理，等待審批，參與中心啟動(dòng)，培訓(xùn)醫(yī)生填寫CRF，原始數(shù)據(jù)核查，解決Query，隨時(shí)報(bào)告不良事件。有的是去清點(diǎn)回收藥品，需要花費(fèi)大量的時(shí)間，很耐心，很細(xì)致。有的是去做SDV，即Source Documents Verification，原始文件核對(duì)。有的是去回收CRF，然后整理齊備交給數(shù)據(jù)統(tǒng)計(jì)部門。有的就是純粹后期的解決一個(gè)又一個(gè)的Query。有的有不少不良事件報(bào)告需要Follow。

目前國內(nèi)醫(yī)療行業(yè)的大環(huán)境不好，導(dǎo)致很多同行和學(xué)弟學(xué)妹都渴望從事CRA（Clinical Research Associate)。

我認(rèn)為在沒有充分了解這個(gè)行業(yè)之前，還是腳踏實(shí)地在目前的崗位上兢兢業(yè)業(yè)，不要一山望著一山高，這樣患得患失反而會(huì)進(jìn)入一種頻繁跳槽卻總也感覺找不到最合適自己的怪圈，最后把心態(tài)也搞壞了。尤其是三十歲以前，是一個(gè)學(xué)習(xí)、積累的過程，其中包括知識(shí)、經(jīng)驗(yàn)和人脈。

我進(jìn)入CRA行業(yè)前準(zhǔn)備了三年多的時(shí)間來了解、參與、實(shí)踐。

首先，CRA不僅需要良好的醫(yī)學(xué)背景還需要對(duì)整個(gè)醫(yī)療運(yùn)作體制的充分掌握，因此很多應(yīng)屆畢業(yè)生直接進(jìn)入CRA行業(yè)很可能無法理解為什么會(huì)有各種各樣層次的教授主任，畢竟在實(shí)習(xí)階段沒有任何利益關(guān)系，就無法真正地融入醫(yī)院的內(nèi)部人文環(huán)境中。

其次，就是對(duì)CRA這份職業(yè)的了解。起初我以為多看國家GCP，國際ICH-GCP就夠了，其實(shí)完全不是這樣。面試的時(shí)候這些最多作為筆試的一些基本常識(shí)，此外更重要地是真槍實(shí)彈地實(shí)踐積累下來后處理應(yīng)急事件的經(jīng)驗(yàn)。

最后一點(diǎn)，應(yīng)該也是最重要的，就是英語，知名的CRO或者藥企中的CRA必須按照項(xiàng)目對(duì)應(yīng)的SOP書寫很多英文報(bào)告，每天工作中可能需要收發(fā)幾十封不同項(xiàng)目來自不同角色的Email，乃至在國際多中心項(xiàng)目時(shí)的電話會(huì)議都要求良好的英文聽說讀寫能力，在這一點(diǎn)上如果英語基礎(chǔ)好的人在面試時(shí)也會(huì)獲得比較高的印象分，這點(diǎn)隨著國際化接軌的加快，在其他行業(yè)也是顯而易見的。

之前看到一個(gè)戰(zhàn)友寫的對(duì)CRA的一些看法，我感到寫得非常中肯，我加入一些我個(gè)人體會(huì)，修改如下，還望戰(zhàn)友不要見怪，旨在交流：

1.CRA前景：CRA 是個(gè)熱門的.職業(yè)；國內(nèi)的起步比較晚，中國的人口數(shù)量多，SFDA的標(biāo)準(zhǔn)有改變，外企重視中國的市場(chǎng)等，所以有比較大的發(fā)展前景。在外資藥企內(nèi)做3－5年就算是資深的CRA； 一般在CRO 公司（正規(guī)的，做多國多中心試驗(yàn)）做了3年都是資深的CRA。

2.CRA薪水：CRA 不是你想象的天堂，賺的絕對(duì)是血汗錢。在大的藥企或CRO，工作非常辛苦，而且強(qiáng)度很大，壓力也很大；盡管出差多數(shù)是出去一兩天，不占用周末和假期，但是timeline壓得根本沒時(shí)間出去，每天都是酒店和醫(yī)院兩頭跑。絕對(duì)會(huì)感覺對(duì)得起你拿的那些薪水；CRO 新人：薪水在5000左右（稅前）。大型藥企多數(shù)有年底Bonus，新人：薪水：7000～9000（稅前）；有3年或以上經(jīng)驗(yàn)的CRA，去了大型外資藥企，平均的薪水：10000（稅前）；待升到Senior CRA通常正規(guī)的CRO：6000～8000（稅前），藥企的高級(jí)CRA也差不多會(huì)多2000左右，但是藥企中很多CRA同時(shí)管項(xiàng)目，可以是CRA也可能SCRA，資深的CRA，可達(dá)到：10000～12000（稅前）；到項(xiàng)目經(jīng)理即Project Manager（在藥企沒有這個(gè)Title），在CRO通?？赡苄枰胄?年到3年，在藥企很可能需要3～5年，薪水都差不多，大約：12000～15000（稅前）；再上升到臨床研究經(jīng)理即CRM及高級(jí)CRM，CRO里大約15000～18000（稅前），大型藥企分工比較細(xì)，會(huì)細(xì)分到事務(wù)、培訓(xùn)、產(chǎn)品安全、產(chǎn)品注冊(cè)、產(chǎn)品醫(yī)生或醫(yī)學(xué)總監(jiān)等崗位，薪水跟工作量以及公司本身都有些關(guān)系。特別指出的是：項(xiàng)目經(jīng)理之后有一些公司除了Salary就會(huì)有項(xiàng)目提成，年底也會(huì)有額外的Bonus，還會(huì)有相當(dāng)不錯(cuò)的Well-Being，除了Social Benefit還有各自企業(yè)不通的Welfare Plan。

3.CRA發(fā)展的空間和曲線： 國外的不少CRA會(huì)在同一家公司一直做到退休，而國內(nèi)相對(duì)就非常少了，然而有一點(diǎn)跟醫(yī)生很相似，就是也有一條很固定的職業(yè)發(fā)展軌跡，是個(gè)很講究專業(yè)的行業(yè)，需要不斷提升自己在該領(lǐng)域的業(yè)務(wù)技能與常識(shí)，此外對(duì)臨床前沿的東西也最好隨時(shí)可以拿起，這樣跟那些知名教授主任討論項(xiàng)目的時(shí)候他們才會(huì)尊重CRA，也會(huì)非常配合保質(zhì)保量地完成項(xiàng)目； 一般的發(fā)展模式是：Junior CRA－Senior CRA－[Project

Leader(PL)、Project Manager(PM)]－Associate manager－Associate director-Clinical Research Manager-Medical Department Manager；如果往BD（Business Development)或Trainer方向發(fā)展也可以，如今很多的PM在管理項(xiàng)目的時(shí)候就會(huì)參與到BD的工作中去，但還是要根據(jù)不同公司的職責(zé)劃分來定。

4.CRA 的適合人群

第一，CRA無論在藥企的醫(yī)學(xué)部還是CRO的操作部都是參與到項(xiàng)目的每個(gè)環(huán)節(jié)，在項(xiàng)目形成雛形的時(shí)候像個(gè)策劃，制定方案的時(shí)候像個(gè)研究生，翻譯方案和知情同意書的時(shí)候是個(gè)翻譯，查閱文獻(xiàn)修改方案通過律師考核的過程像個(gè)督查，成本核算規(guī)劃預(yù)算的時(shí)候像個(gè)會(huì)計(jì)，選醫(yī)院選醫(yī)生的時(shí)候像個(gè)獵頭，進(jìn)入醫(yī)院考察接觸教授主任的時(shí)候像在做課題，遞交倫理的時(shí)候如同個(gè)秘書，組織研究者會(huì)議的時(shí)候像做市場(chǎng)和大型會(huì)議的組織人員，培訓(xùn)醫(yī)院醫(yī)生啟動(dòng)項(xiàng)目的時(shí)候像個(gè)培訓(xùn)專員，招募受試者的時(shí)候又如同一個(gè)銷售，然而核對(duì)數(shù)據(jù)清點(diǎn)材料和藥品的時(shí)候又好像個(gè)公務(wù)員，監(jiān)查的過程要非常細(xì)致如同個(gè)苛刻的審計(jì)或生物統(tǒng)計(jì)，發(fā)生嚴(yán)重不良事件時(shí)又像個(gè)臨床醫(yī)生，在跟蹤不良事件報(bào)告的過程中如同個(gè)安全專員，結(jié)束項(xiàng)目總結(jié)報(bào)告整理材料的時(shí)候又像個(gè)文員，總之親力親為什么都要做；跟各種各樣的人打交道，交際能力要沒有問題。

第二，CRA 要負(fù)責(zé)的事務(wù)會(huì)非常繁瑣并要求極度嚴(yán)謹(jǐn)，你會(huì)感到幾近吹毛求疵的程度，毫不亞于臨床對(duì)病例書寫的規(guī)范要求。很多人做了CRA 后，感覺很有種挫折感，經(jīng)常會(huì)出紕漏，但又不一定是故意的，只是事情太多應(yīng)付不過來。認(rèn)真的工作態(tài)度就可能輕而易舉地處理好這些難題。

第三，CRA會(huì)經(jīng)常出差，如果負(fù)責(zé)的Site（醫(yī)院）多一些，一年中很可能有三分之二的時(shí)間在外面，因此要合理安排好工作與生活，畢竟出差時(shí)間只是一兩天，不影響任何休息日，即便不得以影響了也會(huì)調(diào)休。但出差時(shí)更要工作細(xì)致，一旦有紕漏會(huì)增加成本。此外需要清醒的頭腦去處理很多突發(fā)事件，尤其在外地，Site的管理遠(yuǎn)沒有在本地城市的醫(yī)院容易，時(shí)間都是按照小時(shí)計(jì)算好的，如果效率低或者不仔細(xì)就容易給自己添麻煩。

5.CRA工作的現(xiàn)狀：由于國內(nèi)跟國外臨床試驗(yàn)管理和監(jiān)督方面還有相當(dāng)大的差距，所以如今入行CRA會(huì)經(jīng)常感到許多醫(yī)生還缺乏基本素質(zhì)，往往大城市三甲醫(yī)院從事較多國際多中心臨床試驗(yàn)的教授和主任在這些方面做得已經(jīng)很不錯(cuò)了，可以看到他們會(huì)準(zhǔn)時(shí)在辦公室等候CRA，把事先電話里確定下來的文件準(zhǔn)備好給CRA看，如果有任何問題會(huì)給予耐心的解釋甚至?xí)?jīng)過CRA指正后謙和地分析并商榷解決方法；然而，還存在不少醫(yī)生被以往不良氛圍影響下養(yǎng)成了某種慣性思維方式，或在巨大的生活和工作壓力之下，不理解臨床試驗(yàn)的真實(shí)目的及意義，對(duì)GCP也知之甚少，這樣在接觸中就要求CRA懂得如何去解決這些人為因素造成的阻礙，把不良因素降低到最低限度。具備比較完善的SOP的一些外資CRO或藥企醫(yī)學(xué)部通常還是有比較濃郁的學(xué)術(shù)氛圍，可以在從事CRA的經(jīng)歷中獲得在醫(yī)院永遠(yuǎn)無法獲取的人生閱歷。
