我從事CRA的一點體會

一年前我已從事臨床工作近五年，考了中級職稱后改的行。個中原因主要感到從事醫(yī)生使我顧家太少，盡管我很熱愛這份工作。

醫(yī)生是個造福后代的職業(yè)，自己得到和付出的即便在升到主治后可能略相稱，可也沒有時間安排妥自己的生活。

一年前加入了家CRO，進公司后我先后參與或負責(zé)或參與協(xié)助近5個項目。其中有III期的，IV期的，還有上市后的調(diào)研。Sponsor都是世界藥企前十的幾家。有的項目從選中心開始，組織研究者會議，遞交倫理，等待審批，參與中心啟動，培訓(xùn)醫(yī)生填寫CRF，原始數(shù)據(jù)核查，解決Query，隨時報告不良事件。有的是去清點回收藥品，需要花費大量的時間，很耐心，很細致。有的是去做SDV，即Source Documents Verification，原始文件核對。有的是去回收CRF，然后整理齊備交給數(shù)據(jù)統(tǒng)計部門。有的就是純粹后期的解決一個又一個的Query。有的有不少不良事件報告需要Follow。

目前國內(nèi)醫(yī)療行業(yè)的大環(huán)境不好，導(dǎo)致很多同行和學(xué)弟學(xué)妹都渴望從事CRA（Clinical Research Associate)。

我認為在沒有充分了解這個行業(yè)之前，還是腳踏實地在目前的崗位上兢兢業(yè)業(yè)，不要一山望著一山高，這樣患得患失反而會進入一種頻繁跳槽卻總也感覺找不到最合適自己的怪圈，最后把心態(tài)也搞壞了。尤其是三十歲以前，是一個學(xué)習(xí)、積累的過程，其中包括知識、經(jīng)驗和人脈。

我進入CRA行業(yè)前準備了三年多的時間來了解、參與、實踐。

首先，CRA不僅需要良好的醫(yī)學(xué)背景還需要對整個醫(yī)療運作體制的充分掌握，因此很多應(yīng)屆畢業(yè)生直接進入CRA行業(yè)很可能無法理解為什么會有各種各樣層次的教授主任，畢竟在實習(xí)階段沒有任何利益關(guān)系，就無法真正地融入醫(yī)院的內(nèi)部人文環(huán)境中。

其次，就是對CRA這份職業(yè)的了解。起初我以為多看國家GCP，國際ICH-GCP就夠了，其實完全不是這樣。面試的時候這些最多作為筆試的一些基本常識，此外更重要地是真槍實彈地實踐積累下來后處理應(yīng)急事件的經(jīng)驗。

最后一點，應(yīng)該也是最重要的，就是英語，知名的CRO或者藥企中的CRA必須按照項目對應(yīng)的SOP書寫很多英文報告，每天工作中可能需要收發(fā)幾十封不同項目來自不同角色的Email，乃至在國際多中心項目時的電話會議都要求良好的英文聽說讀寫能力，在這一點上如果英語基礎(chǔ)好的人在面試時也會獲得比較高的印象分，這點隨著國際化接軌的加快，在其他行業(yè)也是顯而易見的。

之前看到一個戰(zhàn)友寫的對CRA的一些看法，我感到寫得非常中肯，我加入一些我個人體會，修改如下，還望戰(zhàn)友不要見怪，旨在交流：

1.CRA前景：CRA 是個熱門的.職業(yè)；國內(nèi)的起步比較晚，中國的人口數(shù)量多，SFDA的標(biāo)準有改變，外企重視中國的市場等，所以有比較大的發(fā)展前景。在外資藥企內(nèi)做3－5年就算是資深的CRA； 一般在CRO 公司（正規(guī)的，做多國多中心試驗）做了3年都是資深的CRA。

2.CRA薪水：CRA 不是你想象的天堂，賺的絕對是血汗錢。在大的藥企或CRO，工作非常辛苦，而且強度很大，壓力也很大；盡管出差多數(shù)是出去一兩天，不占用周末和假期，但是timeline壓得根本沒時間出去，每天都是酒店和醫(yī)院兩頭跑。絕對會感覺對得起你拿的那些薪水；CRO 新人：薪水在5000左右（稅前）。大型藥企多數(shù)有年底Bonus，新人：薪水：7000～9000（稅前）；有3年或以上經(jīng)驗的CRA，去了大型外資藥企，平均的薪水：10000（稅前）；待升到Senior CRA通常正規(guī)的CRO：6000～8000（稅前），藥企的高級CRA也差不多會多2000左右，但是藥企中很多CRA同時管項目，可以是CRA也可能SCRA，資深的CRA，可達到：10000～12000（稅前）；到項目經(jīng)理即Project Manager（在藥企沒有這個Title），在CRO通?？赡苄枰胄?年到3年，在藥企很可能需要3～5年，薪水都差不多，大約：12000～15000（稅前）；再上升到臨床研究經(jīng)理即CRM及高級CRM，CRO里大約15000～18000（稅前），大型藥企分工比較細，會細分到事務(wù)、培訓(xùn)、產(chǎn)品安全、產(chǎn)品注冊、產(chǎn)品醫(yī)生或醫(yī)學(xué)總監(jiān)等崗位，薪水跟工作量以及公司本身都有些關(guān)系。特別指出的是：項目經(jīng)理之后有一些公司除了Salary就會有項目提成，年底也會有額外的Bonus，還會有相當(dāng)不錯的Well-Being，除了Social Benefit還有各自企業(yè)不通的Welfare Plan。

3.CRA發(fā)展的空間和曲線： 國外的不少CRA會在同一家公司一直做到退休，而國內(nèi)相對就非常少了，然而有一點跟醫(yī)生很相似，就是也有一條很固定的職業(yè)發(fā)展軌跡，是個很講究專業(yè)的行業(yè)，需要不斷提升自己在該領(lǐng)域的業(yè)務(wù)技能與常識，此外對臨床前沿的東西也最好隨時可以拿起，這樣跟那些知名教授主任討論項目的時候他們才會尊重CRA，也會非常配合保質(zhì)保量地完成項目； 一般的發(fā)展模式是：Junior CRA－Senior CRA－[Project

Leader(PL)、Project Manager(PM)]－Associate manager－Associate director-Clinical Research Manager-Medical Department Manager；如果往BD（Business Development)或Trainer方向發(fā)展也可以，如今很多的PM在管理項目的時候就會參與到BD的工作中去，但還是要根據(jù)不同公司的職責(zé)劃分來定。

4.CRA 的適合人群

第一，CRA無論在藥企的醫(yī)學(xué)部還是CRO的操作部都是參與到項目的每個環(huán)節(jié)，在項目形成雛形的時候像個策劃，制定方案的時候像個研究生，翻譯方案和知情同意書的時候是個翻譯，查閱文獻修改方案通過律師考核的過程像個督查，成本核算規(guī)劃預(yù)算的時候像個會計，選醫(yī)院選醫(yī)生的時候像個獵頭，進入醫(yī)院考察接觸教授主任的時候像在做課題，遞交倫理的時候如同個秘書，組織研究者會議的時候像做市場和大型會議的組織人員，培訓(xùn)醫(yī)院醫(yī)生啟動項目的時候像個培訓(xùn)專員，招募受試者的時候又如同一個銷售，然而核對數(shù)據(jù)清點材料和藥品的時候又好像個公務(wù)員，監(jiān)查的過程要非常細致如同個苛刻的審計或生物統(tǒng)計，發(fā)生嚴重不良事件時又像個臨床醫(yī)生，在跟蹤不良事件報告的過程中如同個安全專員，結(jié)束項目總結(jié)報告整理材料的時候又像個文員，總之親力親為什么都要做；跟各種各樣的人打交道，交際能力要沒有問題。

第二，CRA 要負責(zé)的事務(wù)會非常繁瑣并要求極度嚴謹，你會感到幾近吹毛求疵的程度，毫不亞于臨床對病例書寫的規(guī)范要求。很多人做了CRA 后，感覺很有種挫折感，經(jīng)常會出紕漏，但又不一定是故意的，只是事情太多應(yīng)付不過來。認真的工作態(tài)度就可能輕而易舉地處理好這些難題。

第三，CRA會經(jīng)常出差，如果負責(zé)的Site（醫(yī)院）多一些，一年中很可能有三分之二的時間在外面，因此要合理安排好工作與生活，畢竟出差時間只是一兩天，不影響任何休息日，即便不得以影響了也會調(diào)休。但出差時更要工作細致，一旦有紕漏會增加成本。此外需要清醒的頭腦去處理很多突發(fā)事件，尤其在外地，Site的管理遠沒有在本地城市的醫(yī)院容易，時間都是按照小時計算好的，如果效率低或者不仔細就容易給自己添麻煩。

5.CRA工作的現(xiàn)狀：由于國內(nèi)跟國外臨床試驗管理和監(jiān)督方面還有相當(dāng)大的差距，所以如今入行CRA會經(jīng)常感到許多醫(yī)生還缺乏基本素質(zhì)，往往大城市三甲醫(yī)院從事較多國際多中心臨床試驗的教授和主任在這些方面做得已經(jīng)很不錯了，可以看到他們會準時在辦公室等候CRA，把事先電話里確定下來的文件準備好給CRA看，如果有任何問題會給予耐心的解釋甚至?xí)?jīng)過CRA指正后謙和地分析并商榷解決方法；然而，還存在不少醫(yī)生被以往不良氛圍影響下養(yǎng)成了某種慣性思維方式，或在巨大的生活和工作壓力之下，不理解臨床試驗的真實目的及意義，對GCP也知之甚少，這樣在接觸中就要求CRA懂得如何去解決這些人為因素造成的阻礙，把不良因素降低到最低限度。具備比較完善的SOP的一些外資CRO或藥企醫(yī)學(xué)部通常還是有比較濃郁的學(xué)術(shù)氛圍，可以在從事CRA的經(jīng)歷中獲得在醫(yī)院永遠無法獲取的人生閱歷。

在擔(dān)任前臺文員一職期間，主要負責(zé)公司內(nèi)部文檔資料的整理和打印，電話的接聽及轉(zhuǎn)接工作，工作期間能夠及時完成好上司分配的工作。
